There are many Health Centres on PEI that offer primary care. Primary care is usually provided by family physicians, nurse practitioners, and other health professionals in the community. Primary care covers a broad range of health services including diagnosis, treatment, education, disease prevention, and screening. Primary care includes physician offices and health centres located in five primary care networks across the province. These community-based networks ensure you have access to primary care services close to your home.
A primary care network is an approach that brings together different health care professionals to ensure you receive the right care, from the right professional, at the right time. You, your family physician/nurse practitioner, and the network team become partners to better manage your health. Each network has a team of health care professionals including family physicians, nurse practitioners, registered nurses, diabetes educators, licensed practical nurses, and clerical staff. Some networks also have dietitians and mental health workers.
